High-risk industries know that we are all fallible and as such we need to learn from the mistakes, errors and violations which we ALL make. To do this there needs to be a Just Culture which allows people to take about their mistakes, free from fear of criticism and punishment when those mistakes are genuine. People (divers) don't get up in the morning and decide "Today is a great day to die!"
