WHY LEARNING REVIEWS?

Learning Reviews aren’t quite the same as traditional incident investigations. This is because their purpose, the way they are conducted, and the focus of the recommendations is primarily focused on learning how normal ‘work’ (diving) is done rather than focusing on non-compliance. Diving that takes place in established domains like the commercial sector or military diving will tend to have such a traditional investigation process. Media diving, scientific diving, and recreational project diving rarely have established investigation processes, and this means that the opportunity to improve is often missed when something goes wrong.

THERE IS A DIFFERENCE BETWEEN 'WORK AS IMAGINED' AND 'WORK AS DONE'


The gaps that exist between 'Work as Imagined', 'Work as Done', and 'Work as Disclosed' mean that a risk exists for the team, the team members, the leadership, and the organisation because there are things going on that are not known, sometimes, not even knowable. The purpose of a Learning Review, is to conduct an activity that highlights these gaps, and provides a means by which they can be closed. Maybe not completely, but at least shine a spotlight on them.
