The presentation I gave at OzTek 2019 focused on the need to understand the difference between Operational Safety and Paper Safety. Fundamentally, just because you are paper safe, it doesn't mean you are operationally safe because there is a limit to what can be covered in any training system or instructor guide. This is only one part of the problem, the other part is that when we are operating in 'knowledge-based' performance, then our error rate is in the order of 1:2 - 1:10, which isn't great odds when you are in a non-life sustaining environment.
